Step 2: Assign a resource from your organization who will be the point person for the project.
Step 3 (Optional): Update your Notice of Privacy Practices (NPP) with a statement disclosing that your organization participates with a Health Information Exchange. You will learn more in Part 3 of the required video series located above. You can also download CliniSync’s Request to Change Consent form below to provide to your patients as well.
Step 4: Download and complete the Community Health Record User Build Form (below).
Step 5: Return your completed Community Health Record User Build Form from Step 4 to your Community Project Manager/CliniSync resource.
