Step 2: Assign a resource from your organization who will be the point person for the project.

Step 3: Complete the DIVA form. This form must be notarized. In the form you will see mention of Health Catalyst. Health Catalyst is a technology vendor to CliniSync. CliniSync will provide your completed DIVA to Health Catalyst on your behalf.

Step 4 : Return your completed and notarized DIVA form by mail and email your Community Project Manager once you are done so they can provide you with the next steps.
