CliniSync HIE Services are designed to enable seamless and secure exchange of patient information within the circle of care. University Hospitals recognizes the importance of having available comprehensive patient information at all points of care. University Hospitals’ and the Quality Care Network’s partnerships with CliniSync make UH acute and ambulatory data available to you and other members of the CliniSync network. Additionally, UH QCN is sponsoring a means for your practice to share summaries of care provided by your physicians and practitioners to support hospital bedside care and better informed medical decision making.

Through this initiative, the following CliniSync Services are available at no cost to you:

  • Continuity of Care Document (CCD) Contribution
  • Continuity of Care Document (CCD) Query
  • Community Health Record (CHR)

The first step is to complete the following CliniSync contracting requirements. Please click on the links below.

  1. CliniSync Physician Participant Agreement and Business Associate Agreement
  2. Statement of Work (SOW) for CliniSync/QCN Participation

Continuity of Care Documents

During scoping of your CCD contribution project, we will also discuss with you the possibility of querying CCDs from within your electronic medical record (EMR) to access additional clinical and demographic information pertaining to your patients.
