The CliniSyncPLUS team has been working closely with the Ohio Department of Health to explore chronic conditions in Ohio, specifically hypertension and diabetes. Working with practices throughout the state, they not only researched the extent of these conditions but suggest ways in which practitioners can manage patient populations who have these illnesses.
