Establishing a Chronic Care Management Program in an Independent Group Practice

We developed the webinar and article with Westshore Primary Care, a group of 60 Ohio providers with locations in Cuyahoga and Lorain counties. Westshore Primary Care established a Chronic Care Management (CCM) program early in 2016. They have focused on enrolling patients with diabetes who require routine care and meet the requirements of the CMM program, or patients who have been discharged from the hospital.
